DVPA Patient Portal

DVPA families may now access to their children's medical records through our convenient, secure patient portal. In order to gain access to the portal, parents or guardians must physically come to our office with a valid photo ID. Once you have signed the necessary paperwork, we will give you your user ID and a temporary password. Once you set up your account, you will be able to access your child's medical records any time of day or night, including: 

• immunization records
• recent physicals
• test results
• sick visits
• medications
• allergies

You can also use the Patient Portal to schedule appointments, communicate with our staff, and pay your bill online!
