Your insurance policy is a contract between you and your insurance company. Please review your policy and become familiar with the benefits it provides.
If we are participating with your insurance company, we will submit any claims from our office on your behalf. However, we can not guarantee payment of your claim. Plans vary greatly in the coverage offered, so the claim may not be paid in full by your company.
If you have any questions about the coverage, you should contact the insurance company or your employer's insurance specialist. If the claim is not covered in full, the balance of the payments becomes your responsibility.
Please contact your insurance company and make our physician your child's Primary Care Physician before your child's first visit. It is necessary that your identification number and eligibility be verified at the time of every visit to our office. Have this information available at all times. It is your responsibility to pay any co-pay at the time of service. Failure to do so will result in your being charged a rebilling fee.
Important reminder: It is always a good idea to double check with your insurance company regarding your policy's coverage before scheduling an appointment with any new healthcare provider.
